Amahte Ibizans Puppy Questionnaire
Like people, each puppy is an individual. This questionnaire will help us select the most suitable puppy for you while
allowing you to clarify your thoughts about dog ownership. It also lets you see what things are important to us in placing
our precious puppies into their new homes.

questionnaire to fill out!!
**I did have an on-line questionnaire but I was having troubles with submissions not being received, so If you filled one
out on-line and did not hear back, its not because I am ignoring you, its most likely due to the fact I didn't receive the
form so I do apologize.
